END ELDER HOMELESSNESS

At a loss about what to manage the overwhelming needs of an elderly homeless patient? Maybe the Elder Shelter to Home Program can help. A service offered through the Committee to End Elder Homelessness helps elders 60 and older (and if resources allow, those 50 - 60) who are homeless. Services provided include case management, housing and benefits application assistance, housing search assistance and stabilization, and support services. On a very limited basis they can also assist elders who are at risk of homelessness. Call 369-1560 to refer. This program is in IRis if you want to give a printout to a patient.
