Boston Partners in Care provides daily in-home NON-SKILLED care to Boston area adults age 22 or older that require personal assistance. They provide monthly case-management home visits, a personal emergency response system if needed and a Personal Care Homemaker (PCHM) services 6 or 7 days a week for 1-2 hours. The PCHM can also offer meal preparation, light housekeeping, medication reminders, occasional errands and more.

Eligibility criteria: on MassHealth or MassHealth eligible, reside in subsidized housing (BHA, congregate housing, assisted living), possess functional or cognitive impairment that prevents the individual from living safely and independently without consistent structured assistance, requires (and accepts) assistance with personal care 6 or 7 days a week.
