Program Highlight:
PrimeCare: MGH Elder Case Management
Entering its second year of operation, Massachusetts General PrimeCare offers elder care management services to elders and those who care for them. Staffed by registered nurses with extensive elder case management experience, the program brings medical expertise and focus in solving the complex daily living issues faced by many elders. Services include, but are not limited to:
- Elder care assessments and evaluations.
- Medical care coordination, prescription management, care advocacy and other medical liaison services.
- Home safety and functionality assessments.
- Assisting elders and caregivers in evaluating and selecting alternative living options, home care supports, etc.
- Finding reliable and trustworthy vendors of needed elder care services.
- Maximizing covered benefits under insurance plans, minimizing premium costs and ensuring cost effective purchases of needed elder care services/equipment.
- Independent and professional guidance for elders/caregivers expressing uncertainty over appropriate levels of elder care support.
- Creating plans and identifying resources to support those caring for elders.
PrimeCare services are available to MGH patients and are discounted for MGH employees/families. Although most insurances do not cover geriatric care management services, some long-term care insurance policies do include such a benefit.
