PROGRAM HIGHLIGHT: HOMEPOWER- EXPANDED HOME CARE FOR ADULTS
HomePower, also called Group Adult Foster Care, makes it possible for disabled individuals age 22 and over, including seniors, who live in designated public or subsidized housing to continue living safely at home. HomePower is a MassHealth-funded program of Boston Partners in Care, a partnership between Boston Senior Homecare and the Visiting Nurses Association of Boston. This program accesses the client’s MassHealth Group Adult Foster Care benefit.
Services
Services include:
- Personal Care homemaker (PCHM) one hour 5-7 days/week, or more depending on need. A PCHM performs direct personal care, such as a sponge bath, shower, foot care, assistance with dressing and/or grooming and help with transfers and ambulation. Other duties may include preparing meals, light housekeeping, running errands and medication reminders.
- Monthly visits by program registered nurse or case manager
- Personal Emergency Response system (such as “Lifeline”).
Visiting nurse and/or therapist services from an outside agency may continue. These services may be long-term to serve the needs of those with chronic health conditions or permanent disabilities. Clients do not need to be homebound.
Additional Services MAY include (depending on need):
- Adult Day Health program 2-5 days/week
- Home Delivered Meals
- Grocery shopping
- Homemaking
- Laundry
- Transportation
Eligibility
- Age 22 or older
- Currently receive MassHealth Standard or MassHealth CommonHealth
- Live in a managed building (BHA, congregate housing, senior housing, etc.)
- Have difficulty in activities of daily living such as bathing, dressing, grooming, walking; preparing meals, housekeeping, or taking medications
- Requires and accepts assistance with personal care 5-7 days/week
- Lives in Boston or other local community accessible by public transportation
