NEW ADULT GUARDIANSHIP MEDICAL CERTIFICATE

As of April 15, 2008 The Massachusetts Probate and Family Court is requiring use of a new, revised medical certificate form for adult guardianship. The new form is now on our website. Please discard all old paper copies. If staff have never pursued guardianship, please first contact Karen Tanklow or Karon Konnor for guidance.
