CeltiCare SNF Benefits & Network

 

Here’s some clarification about CeltiCare’s SNF benefits and provider network as it relates to Partner’s facilities.

CeltiCare Bridge: The Partners SNFs are not included in our CeltiCare Bridge agreement, due in part to the fact that Bridge members don't have any SNF benefits, only 100 days of inpatient rehab per calendar year - no SNF.

CeltiCare Commonwealth Care: The Partners SNFs are also not included in our CeltiCare Commonwealth Care agreement. Commonwealth Care members have 100 days of inpatient rehab and/or SNF benefits per calendar year. Note that the benefits are different from those offered in the Bridge program.
