PROGRAM HIGHLIGHT:
PARTNERS HOMECARE LIFELINE

Partners Home Care now has a contract with a personal emergency response system service- Lifeline. Any patient of Partners entities is eligible for Partners Home Care Lifeline rates which may be lower than average. Rates include a one-time installation fee of $45 and monthly fee of $35 or $45 a month, depending upon the equipment needed. The higher-end equipment includes a feature that allows the patient or loved ones to record up to 6 verbal reminder messages that will play at predetermined times and can be used to prompt patients to take medications, elevate their legs, go to appointments, etc.

The Lifeline set-up turnaround time is within 24-48 hours, though on a case-by-case basis they may be able to accommodate a quicker turnaround.

Medicaid may cover the installation and monthly fee for the standard model. Lifeline will educate medical staff about the need for documentation of medical necessity, remind them as necessary and see the process through completion.

The service area is the Partners Home Care service area- most of Eastern Massachusetts (East of Worcester).

To refer, staff should use the Partners Home Care designated Lifeline referral line- 800-910-4225. We also have a designated Partners employee who is the liaison to this program. Her name is Yinh Kiefer. She is housed at Partners Home Care in Beverly and can be reached via e-mail or at 978-236-1395 for questions about the service.
