Description
The Department of Clincial Social Work, in collaboration with the Department of Nutrition and Food Services, provides a supply of easy to prepare, nutritious, familiar and tasty foods - enough for 6-8 meals - for financially needy patients with known limited food supply at home who meet additional criteria.
Eligibility
Eligibility is based on both the client’s financial situation and access to food. Social workers make the decision about eligibility, but need to complete the form so that the department can track usage.
Please note: only for patients already onsite.
Benefits
Patients receive enough food to make 6-8 nutritious meals.

Instructions
1. Call x4-3663 to order and arrange for on-site delivery (or pick-up at Blake basement) AT LEAST one hour’s notice is required, but as packs are assembled as needed, as much notice as possible is appreciated. Hours: 8:00 a.m. to 7:30 p.m.
Please note this program is only for patients already onsite.
2. Complete form. If ALL eligibility criteria are not met, please explain extenuating circumstances. Make a copy of the completed form.
3. Give the original, signed form to the staff member who delivers the meal.
4. Send the copy of the form to Ellen Forman via house mail. No pre-approval required.
5. Please refer to appropriate community resources as soon as possible.
